Do I have to pay the $5 fee and obtain another form from the CVSO each time I renew my DL/ID card?

No, the Veteran designation should permanently remain on a person's driver license or identification (DL/ID) card.  A person will be able to renew by mail or Internet and there will not be a question that asks them if they want the Veteran designation, but the DL/ID card will be produced with the designation, if it was on his or her prior DL/ID card.  If a person renews his or her DL/ID card in a DMV field office, the person will have to mark on the DL/ID application that they want to retain the Veteran designation.  The person will not need to submit another VSD-001 form to DMV.
